You may submit information requests or questions that are non-urgent, non-confidential and non-medical in nature. You should expect a reply within five regular business days during daytime hours.

We are not taking on new patients or granting any appointments.

The location is virtual but our head office is located in the Fredericton area.

We do not offer 24/7 access for any of our services, for this reason, there can be a delay in response to our submission form on our website and or our secure messaging system. Our availability is limited and on a variable and weekday part-time schedule, please check our website banner for closure notices as the office may be closed from time to time.

We do not respond to messages after hours, weekends or statutory holidays and only on selected weekdays when we are open, so please do not submit any urgent messages.

During closures, after hours/evenings, weekends or holidays, once we reopen we will respond in a timely manner but this may take a few regular business days to do so.


How can I access my Access Point Health medical records?

You must request the AUTHORIZATION FOR RELEASE OF PERSONAL HEALTH INFORMATION form and this form must be properly and completely filled to be processed.

Once Access Point Health has received the AUTHORIZATION FOR RELEASE OF PERSONAL HEALTH INFORMATION form that has been fully and properly completed and signed, Access Point Health has up to 30 days to provide you with the medical information requested as per PHIPPA. (This form can be requested and sent back to us via our designated fax line 1 (855) 821-4007 or by mailing it to ACCESS POINT HEALTH P.O. Box 30073 Prospect Plaza, Fredericton, N.B. E3B 0H8 Canada (Please see our fees - mailing fees do apply for having medical information sent back by post)).

What if I am requesting a medical record for a child/minor from Access Point Health?

The legal guardian must be the one submitting the request for this medical record.  In the case of a mature minor, the mature minor must also sign and consent for the release of medical information to occur.

Can I ask for certain information to be kept private and not shared?

Yes, in most circumstances for certain information to be kept private. We do however, have a legal duty to report if we suspect child abuse or if we are made aware that someone may pose a threat to himself or herself or someone else or that we are concerned with a person’s competence and medical capacity in decision making processes (i.e. you are too unwell to make the rational decision you would make when well).
